Check out the following two sample mid-semester survey questionnaires that has both open-ended and likert-scale questions:  

Import sample Canvas surveys into your course site 

  1. In Course Navigation, click the Settings link.
  2. Click the Import Course Content button.
  3. In the drop-down menu for Content Type, select the QTI .zip file option.
  4. Choose File, select the downloaded ZIP file you want to import and Click Open.
